Requirements to Replace Lost, Stolen, or
Mutilated License Plates

Plate(s) that have been stolen, or damaged/mutilated for the purpose of defacing the plate, must be reported immediately to your local police department or resident state trooper. 

Once you have obtained a written police report, you must bring the police report to your local DMV full service branch or satellite office along with:

  • Remaining plate(s), (if any)
  • Registration Certificate
  • Connecticut Insurance Card

You will complete a Lost or Stolen Marker Plate (form E-159) and a Registration Affidavit (form B-301).  Two off the shelf plates will then be issued to you for $5. Without the police report, the fee for the new plates will be $25. Case numbers will not be accepted in lieu of the police report.

For replacement of a lost license plate(s), go through the same procedure as above; however, no police report is required. The fee for replacement of lost license plates will be $25.

Note: Any plate that is lost or stolen will be replaced with a new set of off-the-shelf plates. You must then wait 10 months if you choose to re-apply for the original plate number. Please complete and submit a Replacement Plate Application (form E-45) with the appropriate fee.

Vehicles cannot be registered if delinquent property tax or parking tickets are owed or if the registrant has had their registration privilege suspended.
